The Yale-Griffin Prevention Research Center and incentaHEALTH partner to offer Type 2 diabetes prevention

The Yale-Griffin Prevention Research Center is working with incentaHEALTH to offer incentaHEALTH’s virtual diabetes prevention program, #PreventDiabetes, to Connecticut community members. #PreventDiabetes is a Centers for Disease Control (CDC) fully recognized virtual National Diabetes Prevention Program.

The Yale-Griffin Prevention Research Center is conducting a research project working to improve the health in low income communities by delivering a virtual Diabetes Prevention Program, #PreventDiabetes, in coordination with a community care coordinator. The goal is to remove the barrier of an in-person Diabetes Prevention Program and decrease disease risk. Learn more about the research project here.

The Yale-Griffin Prevention Research Center (PRC) is a part of the CDC’s Prevention Research Centers network of academic research centers across the United States. All PRCs work with community partners to develop, implement and evaluate community-based approaches to help prevent and control chronic diseases.

The incentaHEALTH #PreventDiabetes program is a year-long virtual lifestyle change program. Individuals can reduce their risk for developing Type 2 diabetes with the support of video sessions, app-based coaching with exercise plans and meal ideas, patented photographed progress tracking, cash incentives for weight loss, the support of a certified Diabetes Lifestyle Coach and more.

incentaHEALTH works with individuals, employers, physician groups, health systems and more to offer evidence-based weight management and diabetes prevention solutions. Contact us to learn more and schedule a demo.
